All Officials must use the USA Hockey Game Incident Reporting System for ALL game levels!

**PLEASE SEND A COPY OF ALL INCIDENT REPORTS TO KYLER HARRIS AND MIKE MCGARRAH** 

​

This reporting format is to be used for ALL level of games for both youth and adult hockey.  Any official calling the following penalties must fill out a game report within 48 hours of the penalty by following the instructions below. Please fill out your game report as soon as possible following the conclusion of your game so any appropriate suspension can be handled promptly. 

 

  1. All Match Penalties

  2. All Game Misconduct Penalties

  3. All Major Penalties

  4. All Special or unusual situations that require action by the proper authorities

*** ACHA Incident Reports need to be filed for ALL GAME MISCONDUCT and DISQUALIFICATION penalties.***

Use the ACHA Incident Report (word) (pdf) and follow the instructions. Report must be submitted to appropriate ACHA commissioner(s) within 48 hours of incident.
